Preservation Rhinoplasty is a surgical approach to nose reshaping that aims to maintain and preserve as much of the patient’s natural nasal structure as possible. The technique is designed to produce more subtle and natural-looking results compared to traditional rhinoplasty procedures that involve extensive removal or reshaping of nasal cartilage and bone.
In Preservation rhinoplasty, Dr. Lyor Hanan carefully evaluates the patient’s nasal anatomy to determine the areas that require modification. After that, he tries to reshape the nasal components that are already there without removing or changing them needlessly. With a lower chance of complications like nasal collapse or respiratory issues, this approach can result in a more stable, durable, and functional nose, with less risk of complications such as nasal collapse or breathing difficulties.

His formal qualifications include a D.E.S. in General Surgery (Université Paris 7 – Bichat), a D.E.S.C. in Plastic, Reconstructive and Aesthetic Surgery (Université Paris 7 – Bichat), a D.U. in Aesthetic and Functional Rhinoplasty (Université Paris 13 – Sorbonne Paris Nord), a D.I.U. in Applied Anatomy for Plastic Surgery (Université de Bordeaux), and a D.U. in Microsurgery (Université Paris 5 – Paris Descartes).
Dr. Lyor Hanan specializes in natural-looking facial rejuvenation and advanced breast surgery, including Deep Plane Facelift, upper and lower blepharoplasty, aesthetic and functional rhinoplasty, breast augmentation, breast lift, breast reduction, and facial and breast reconstruction. He is also certified to perform the MIA Femtech procedure.
Combining aesthetic judgment with reconstructive expertise, Dr. Lyor Hanan plans each treatment around the patient’s anatomy, goals, safety, and recovery, aiming for refined and harmonious results that preserve identity.
The principle of preservation rhinoplasty is to maximize the preservation of your nose’s features while correcting only the main defects such as a prominent bump or an oversized nose. This technique is particularly reserved for patients who have complaints about their ¾ or profile view, but little to no concerns about the front view.
Basically, instead of removing the bump from above (as in standard rhinoplasty technique) and then trying to reconstruct everything identically, the bump is removed from underneath, preserving the unchanged nasal roof. The whole job is done from underneath your actual nasal roof anatomy to preserve it.
Preseveration Rhinoplasty offers numerous benefits, including:
Because it requires less cutting and reshaping of the nasal tissues, preservation rhinoplasty is a less invasive surgery than regular rhinoplasty. As a result, there may be reduced discomfort, bruising, and swelling following the procedure.
Compared to regular rhinoplasty, preservation rhinoplasty frequently yields more natural-looking outcomes. A more balanced and harmonious appearance can be achieved by the surgeon by maintaining the patient’s natural nasal structures, preventing the “operated-on” appearance that might arise from extensive nasal contouring.
While standard Rhinoplasty includes removing or reshaping nasal cartilage and bone to obtain the desired result, Preservation Rhinoplasty seeks to maintain the patient’s natural nasal features as much as possible.
Compared to normal rhinoplasty, preservation rhinoplasty has a lower chance of consequences such infection, respiratory problems, and nasal collapse. The surgeon can produce a more stable, long-lasting nose that is less likely to have these kinds of problems by maintaining the patient’s original nasal architecture.
For patients looking for a more conservative kind of nose reshaping, Preservation Rhinoplasty is a great choice overall. However, based on their unique nasal anatomy and objectives, it might not be appropriate for every patient. To choose the best course of action for your needs, it is imperative that you speak with a skilled and knowledgeable cosmetic surgeon, like rhinoplasty specialist Dr. Lyor Hanan.
Preservation rhinoplasty is a more complex technique than standard rhinoplasty, with the main goal being to remove the bump from underneath rather than from above.
The skin envelope is minimally elevated during preservation rhinoplasty, via a minimal incision at the columella. Thus, the post operative bruises is less common.
The dorsum, or bridge of the nose, is lowered and flattened to correct the bump.
The alar cartilages (responsible for the shape of the nasal tip) are carved, refined, and supported by a small piece of cartilage to prevent a drooping tip.
The procedure takes place in an operating room within a private surgical hospital.
The first step involves administering general anesthesia and clean the patient’s entire face and nose.
The second step is the incision. Dr. Lyor Hanan uses both closed techniques with a hidden incision inside the nose, and open techniques where a discrete incision is made at the columella (the strip of skin separating the nostrils). Through this incision, the nose skin is gently lifted to reshape the cartilage.
The third step is working on the bump, which is done underneath by removing excess cartilage and bone from below, not from above, to preserve maximum characteristics of your nose. The bones are then delicately cut using specific minimally invasive instruments.
The fourth step is reshaping the nasal tip. Excessive cartilage is meticulously removed while maintaining sufficient cartilage height to prevent nostril retraction. Sutures are then placed between the cartilages to refine them.
The last step is the final skin and internal sutures. This closure is performed using dissolvable sutures that disappear after a few days to weeks.
Recovery after preservation rhinoplasty is generally straightforward, with little pain after rhinoplasty. Although Dr. Lyor Hanan doesn’t use post-operative packing, it’s mainly the sensation of nasal congestion (like a cold) that is uncomfortable. These symptoms resolve quickly within a few days.
Post-operative recovery after preservation rhinoplasty is simpler than with classic rhinoplasty because the technique is less invasive.
The goal of preservation rhinoplasty are to correct nasal defects while maintaining your nose’s natural features, preserving maximum initial structures to achieve the most natural result possible.
The price of preservation rhinoplasty is around 40 000 AED.
